The study itself is usually sound. What comes back is the paperwork, returned for reasons that feel painfully small when you finally read them: a figure that changed while you were still thinking, a promise in the consent form that the questionnaire quietly breaks.

02The board's form, answered in the board's order

Every board publishes its own application, and the fields are not decoration. They are the order in which a reader thinks, so an answer written in the shape of your proposal rather than the shape of the field creates work for somebody who has fourteen other studies to get through that month.

The protocol therefore gets composed inside your board's template from the start. Where a field wants two lines it gets two lines. Where it wants a procedure, it gets steps in sequence. Nothing is imported from a generic document and trimmed to fit, because that is exactly what a second round is made of.

03The letter, answered item by numbered item

A revision letter looks like a rejection and is nothing of the sort. It is a list of conditions, and each condition has a small, specific thing that would close it. Studies stall because the letter sits unopened for a fortnight, not because the conditions were hard.

The response goes back in the letter's own numbering. Each item gets a reply saying what was changed, where the change now sits, and the new wording quoted so nobody hunts for it. Attachments are updated together so nothing contradicts anything else, which is what a second letter is usually complaining about.

04What is not promised, and what runs alongside

Nobody can promise approval, and anyone who does is selling you something that is not theirs to sell. What can be promised is a submission that is complete, internally consistent and written where the board reads, which is what removes the avoidable rounds.

Can approval be guaranteed?

No, because the decision belongs to a committee nobody here sits on, and any promise about it would be worth nothing. What can be controlled is completeness and internal consistency, which is where the avoidable rounds actually come from. A set of documents that agree with each other, written into the board's own form, clears fastest.

Only my revision letter needs answering. Is that a job you take?

It is, and people ask for it more than anything else on this page. Send the letter along with everything you originally submitted. Each numbered condition gets a specific reply, the affected documents are updated so they still agree with one another, and the response goes back in the order the board wrote it.

Is my project research, or is it quality improvement?

That distinction decides which route you take and sometimes whether you go to the board at all, and it turns on intent and generalisability rather than on the size of the study. It is settled first, with your own institution's definitions in front of us, because guessing it wrongly costs weeks either way.

Are the consent forms and instruments written too?

They are, and deliberately at the same time as the application rather than afterwards. A consent form written later tends to promise something the protocol did not describe, and an instrument built last often asks a question nobody was consented for. Written together, those contradictions never appear in the first place.

Who does this work, and what have they done before?

Specialists holding research doctorates who have taken their own studies through review and, in several cases, sat on the reading side of a board. That matters because the job is anticipating a specific reader's objections, which is learned by having been that reader rather than by reading a guidance page.
